Vital Records
The Registrar maintains and provides copies of death records for St. Francis Hospital and deaths which occurred within the Village of Flower Hill.
Death Records:
Records for persons who died in the Village of Flower Hill may be requested by the immediate family (spouse, children, parents, siblings) or by other persons who can document a legal or medical need. Applicants applying in person should bring their photo identification, proof of relationship to the decedent, and the appropriate fee of $10.00 per copy to the Registrar, Village of Flower Hill, 1 Bonnie Heights Road, Manhasset, NY 11030. Applicants will be asked to complete the “Application to Local Registrar for a copy of Death Record” form.

A request for Death Records may be accepted in writing with the appropriate fee under the following conditions:
- The applicant is qualified as stated above and provides the same information requested as when applying in person.
- The applicant must also provide the name of the deceased and the date and location of death.
